READ THE FOLLOWING INFORMATION

The Transferor, a Mack Factory Branch Subsidiary, Mack Trucks, Inc. (as successor to Mack Financial Corporation by merger), Associates Commercial Corporation, Mack Leasing System, Inc., a duly authorized Mack Distributor or the original retail purchaser of the motor vehicle manufactured by Mack Trucks, Inc. and bearing the Truck Model and Vehicle Identification Number ("Mack Vehicle") shown above, herewith requests the approval of Mack Trucks, Inc. for the transfer of the Mack Standard Warranty: On-Highway Vehicles or Vocational, On/Off Highway Vehicles and the Supplemental Warranty Covering Emissions Control Systems ("Mack Warranty") to the above purchaser ("Purchaser") of the Mack Vehicle.

The date of the sale of the Mack Vehicle from the undersigned to the Purchaser shall be as shown above. The Transferor herewith warrants and covenants that he (she/it) has complied in all respects with the provisions, terms and conditions of the Mack Warranty and of the Company's Warranty Policy as set forth in its Warranty Administration Manuals andthat the Mack Vehicle has been maintained and operated fully in accordance with the Mack Warranty. The total miles of operation on the Mack Vehicle as of the date of the sale of the Mack Vehicle to Purchaser is as shown above. Purchaserhereby covenants and acknowledges that he (she/it) has herewith received a copy of the Mack Warranty, has read it and fully understands the Purchaser's rights and remedies, and Mack Trucks, Inc.'s obligations, thereunder.

The above Representative, as a representative of the above named Mack Dealer, does hereby certify that the above information is true and correct. The Representative also certifies that the vehicle will be used in the service specified above.

The Warranty Transfer does not take effect until this form is received and approved by Mack Trucks Warranty Department.
